To Benefits Eligible Staff,
At Rochambeau, the French International School, we value our employees and are committed to providing you and your family with affordable health benefits that meet the requirements of “minimum essential coverage” under the Affordable Care Act (ACA).
In the coming days, Form 1095-C is being provided to you as proof of the health coverage we offer you and your family. In addition to information about the health coverage that was offered to you, it also lists the lowest monthly premium that you could have elected for the self-only health benefits we offered to you. Employees who will receive the form include those who:
∙ were enrolled in healthcare coverage through Rochambeau at any point in 2016, or
∙ worked an average of 30 or more hours per week or were considered a full‐time employee (as defined by the Affordable Care Act or “ACA”) in 2016.
You may already have received Form 1095-B from the health insurance company provider enrolled in last year. It contains information about who provided your health insurance, as well as which members of your family covered by the policy and the months of the year each person was covered.
You will need both Form 1095-C and Form 1095-B to help you report your insurance coverage when filing your tax return. Only one of each form is provided for all the individuals listed on your policy; you may need to provide copies to your spouse or dependents, as necessary.
